Fire Service Accident/Incident Reporting form

The Department of Home Affairs – Fire and Rescue Service has a statutory requirement to report all workplace accidents and dangerous occurrences. This online form must be completed for all Safety events including Accidents, Dangerous Occurrences and Near Misses. Each submission will be sent to the Fire and Rescue Services Health and Safety Officer for review and investigation.
